New York State Department of Health

Achievement of Guidelines-Based Medical Management

The Lung Association is partnering with the New York State Department of Health to deliver our asthma quality improvement program, Project BREATHE NY. Project BREATHE NY aims to improve health outcomes and quality of life for children with asthma and their families and reduce asthma related emergency visits and hospitalizations among children. Project BREATHE NY is an innovative quality improvement (QI) program that expands the delivery of quality, guidelines-based asthma care across health systems in New York State. Project BREATHE NY provides a comprehensive framework to engage health systems (in-patient, emergency department, and primary care) and community-based partners in integrating sustainable, evidence-based strategies and best practice tools across settings to educate patients and providers, drive QI in asthma care, build clinical-community linkages, and address health equity and social determinants of health.

NYC Department of Health
