Thirty-five states and D.C. have expanded their state Medicaid program under the Affordable Care Act. States are required to provide these newly enrolled individuals coverage of the essential health benefits, including preventive services. Tobacco cessation is a preventive service and is required to be covered for all Medicaid expansion enrollees without cost-sharing, including co-pays. Despite this, there continues to be inconsistent coverage of tobacco cessation treatment for the Medicaid expansion population. More than half of the hospitals in the US are private, nonprofit organizations that are tax-exempt. Since 2012, they have been required to conduct Community Health Needs Assessments every three years and implement a strategy to deal with issues identified by that Needs Assessment. Data indicates that average spending by tax-exempt hospitals on community benefits has not changed much since 2012. Most hospitals have completed just one CHNA cycle and are struggling with how to have the most impact within the limits of their resources and expertise. Over two-thirds of adult smokers in the United States say they want to quit. A little over half of smokers received advice to quit from their healthcare provider, but only 10 percent of smokers quit successfully in 2015. By integrating tobacco cessation into the fabric of health systems, those numbers can increase. These systemic changes can be daunting to undertake.
